Many execution leaders suffer gravely from previous errors, mistakes, dangers, pitfalls and losses as a result of overlooking oversight responsibilities. Oversight failure is a grievous error which occurs when a leader fails to pay attention to details, neglect to carry out root cause analysis of failures and indulge in a make-believe myth that all things are going well because they have competent subordinates managing processes.

The cost of oversight failure is usually expensive, avoidable and condemnable by those who audit and superintend over the leader who fails to do the needful.
